You and Your General Practice
From 1 October 2025, it is a contractual requirement for all GP practices to publish on their website a Patient Charter called “You and Your General Practice (YYGP)”.
The charter outlines what patients can expect from their practice and how patients can help the practice deliver the best care.

You and Your General Practice - Patient Charter
This page provides information about the NHS You and Your General Practice (YYGP) Patient Charter, which outlines what you can expect from our practice and how you can assist us in delivering the best care for you and our community.
The charter is designed to support a positive relationship built on mutual respect and understanding. It encourages patients to be on time, be prepared for appointments, and cancel appointments they no longer need. It also describes how the practice can support patients (e.g., through reasonable adjustments) and how you can provide feedback or raise concerns.
