New Patient Registration

We have an open list and welcome requests for registration from patients living in or moving to the Practice area which serves Cranborne, Verwood, Alderholt and the surrounding areas within a radius of about 8 miles.  Please note if your address falls outside of the practice catchment area you will be advised that you are out of area.

You can register by bringing along your NHS medical card or completing our forms which are available from reception in our New Patient Pack or click here for the Registration Form (GMS1) and click here for a new patient health questionnaire. 

You will be officially registered with the Practice, and may see whichever doctor you choose; however we would encourage you to consider one doctor as “your” doctor and to see that one on regular or non-urgent visits. Of course for emergency or urgent visits you will need to see whichever Doctor is available at the time.

We recommend that new patients undertake a health check with a nurse; please speak to reception if you wish to make one of these appointments.
