This important part of our team takes care of patients in their own homes when it is impossible for them to attend the surgery. They are registered nurses with a period of at least two years qualified experience. They have also undertaken a specialist qualification in community nursing. They offer professional and confidential advice and support to patients, their families and carers. They offer skilled nursing care for acute, chronic, terminally ill patients and patients who are discharged from hospital. Advice can be offered through a period of illness and how to obtain assistance from other agencies e.g. benefits etc.
